After entering the required inputs, the tool uses a validated model to calculate personalised estimates of 2-week and 10-week mortality risk for individuals with HIV-associated cryptococcal meningitis. We provide a ‘basic’ model, using routinely measured predictors to enable use in resource-limited settings, and a ‘research’ model for use in research settings, where additional clinical information may be available. All predictors should be measured on the day of hospital admission.

PERISKOPE-CM is currently for research use only. The models were developed and validated using data from two randomised-controlled trials conducted in sub-Saharan Africa.

The research underlying development and validation of PERISKOPE-CM is currently under peer-review. The preprint is available here.

PERISKOPE-CM was developed using individual-level data from two randomised-controlled trials of HIV-associated cryptococcal meningitis across eight sub-Saharan African countries (ACTA, ISRCTN45035509; AMBITION-CM, ISRCTN72509687). We used this pooled dataset to develop and validate multivariable logistic regression models for 2-week and 10-week mortality.

To generate ‘basic’ model predictions, the following predictors are required: age, weight, ECOG performance status, Glasgow coma scale, presence of seizures, haemoglobin and neutrophil count.

To generate ‘research’ model predictions, the following predictors are additionally required: CSF quantitative fungal culture, CSF opening pressure and CSF white cell count.

Full methodological details can be found in our preprint here. The PERISKOPE-CM tool assumes that standard treatment is provided according to WHO guidance – including antifungal therapy, monitoring for drug toxicities and management of raised intracranial pressure.
